What is Male BBL surgery?
Male BBL uses the same fat transfer technique as a standard BBL — liposuction removes fat from donor areas, the fat is processed, and it is injected into the buttocks. Because the procedure uses your own fat rather than implants, the result feels natural and the liposuction component provides simultaneous body contouring.
The male aesthetic goal is meaningfully different from the female BBL. Female patients typically want a rounded, hourglass shape including hip enhancement. Male patients want a squarer, more upright shape — greater projection from the rear profile, without the hip widening that would feminise the appearance. Fat is placed in the upper and central buttock zones only; lateral hip injection is specifically avoided.

The result is a lifted, projected buttock with a straight vertical line from the hip to the thigh — consistent with a masculine physique and avoiding any curved or hourglass silhouette.
Some male patients — particularly those who train regularly — prefer a rounder outcome with more volume distributed across both the upper and lower buttock quadrants. This requires more donor fat and a different injection map. Your surgeon will discuss whether your anatomy supports this goal.
Fat is never injected into the lateral hip or outer thigh in male BBL. Hip widening feminises the silhouette and is inconsistent with male aesthetic goals. Your surgeon will plan the injection map at consultation accordingly.
The Male BBL procedure
The Male BBL is performed under at our as a day-case procedure. The procedure takes two to four hours depending on the number of liposuction areas.
Fat is removed from the donor areas agreed at consultation — typically the abdomen, flanks, lower back, and sometimes the banana roll beneath the buttocks. Centre for Surgery uses as standard. For patients combining Male BBL with , VASER ultrasound-assisted liposuction is used. Incisions are 3–4mm, placed in natural skin folds.
The harvested fat is processed using the SAFElipo method — saline washing and centrifugation to remove blood, fluid, and non-viable cells. This maximises viable cell count and improves the retention rate after transfer.
The processed fat is injected into the agreed buttock zones using blunt-tipped cannulas and the EVL (expansion vibration lipofilling) technique. Fat is always placed above the gluteal muscle — never into or beneath it. Real-time ultrasound guidance is used throughout to confirm subcutaneous placement.
For male patients, fat is placed in the upper and central buttock zones only. Lateral hip injection is specifically avoided.

Recovery after your Male BBL
Recovery after Male BBL follows the same pattern as a standard BBL. The most important factor — and the one with the most direct impact on your result — is strict adherence to the no-sitting rule in the first two weeks.
Do not sit directly on the buttocks for the first two weeks. Direct pressure compresses the transferred fat cells before they have established a blood supply, reducing survival. Sleep on your front or side. Use a BBL cushion when you must sit.
Wear your compression garment continuously for six weeks, removing only to shower. This reduces swelling in the liposuction areas, supports the skin as it contracts over the new contour, and reduces the risk of seroma formation. Compression garments are not included in procedure costs.
Most patients with desk-based roles return to work from one week. Patients with physically demanding roles should allow two weeks minimum. Avoid driving for two to three weeks. The gym and strenuous exercise can resume from six weeks.

What results can I expect from a Male BBL?
Results develop over six months as post-surgical swelling resolves and the surviving fat integrates.
The buttocks will appear swollen and larger than the final result. The liposuction areas will look slimmer immediately. Do not assess the outcome in the first weeks.
Residual swelling resolves and the transferred fat softens. The shape of the posterior becomes clearer and the liposuction areas settle into their new contour.
Your body reabsorbs 30 to 50% of transferred fat during this period — this is normal and planned for. By six months, the surviving fat has integrated. The combination of a leaner midsection and improved posterior projection is usually most apparent when viewed from the rear and side profile.
Fat that has integrated at six months is permanent. Weight gain increases buttock volume; weight loss reduces it. Maintaining a stable weight is the most important factor in preserving the result long-term.

What To Expect
At your consultation at Centre for Surgery, your surgeon will assess your fat distribution across potential donor areas — typically the abdomen, flanks, and lower back — and examine your buttocks to assess starting shape, skin quality, and the specific zones for fat placement. The aesthetic discussion is particularly important for male patients. Your surgeon will ask about your goals — whether you want to address a flat appearance, improve projection, correct asymmetry, or achieve a more athletic overall shape — and will discuss the H-shape versus rounded options and what is achievable given your donor fat volume. If you are interested in combining Male BBL with HD liposuction or abdominal etching, this is discussed at the same appointment. You will be told directly if the procedure is not appropriate for your anatomy. Your surgeon will also discuss buttock implants if donor fat volume is insufficient for a meaningful fat transfer result. A written cost quotation is provided. Once you have decided to proceed and the cooling-off period has passed, our preoperative assessment team will confirm your medical fitness before surgery. Stop smoking at least four weeks before your procedure and for a minimum of four weeks after. Smoking significantly reduces fat graft survival by impairing blood supply to the transferred cells, and increases wound healing complications. Stop aspirin at least one week before surgery. Stop ibuprofen and other NSAIDs unless directed otherwise. Stop supplements affecting bleeding — fish oil, vitamin E, ginkgo biloba, garlic, St John's wort. If you take oestrogen-based medications, discuss at consultation. Reduce alcohol and stop for one week before surgery and one week after. Do not eat for six hours before your procedure. Clear fluids only (water, black tea or coffee without milk) up to two hours before. Your admissions team will confirm exact timings. Arrange a responsible adult to collect you and stay with you for the first 24 hours. Driving is not permitted on the day. Prepare your recovery space with extra pillows — you will need to sleep on your front or side and avoid sitting directly on the buttocks for the first two weeks.

Your consultant anaesthetist will confirm fitness for TIVA anaesthesia. Your surgeon will take final measurements and photographs and mark the donor areas and injection zones with you before you go to theatre. The procedure takes two to four hours depending on the number of liposuction areas. Stage 1 is liposuction of the agreed donor areas using PAL Lipo or VASER. Stage 2 is fat processing using the SAFElipo method.
